Cadence Chiropractic is required by law to maintain the privacy and security of your Protected Health Information. This Notice of Privacy Practices describes your rights and explains how we may use and disclose your health information for treatment, payment, healthcare operations, and other purposes permitted or required by law.
This Notice is available to you at any time here on our website, and you may request a paper copy from our office.
YOUR RIGHTS:
You have the right to inspect or obtain copies of your records. We may charge a reasonable, cost-based fee for copies as permitted by law.
You have the right to request amendments to your health information if you believe it is incomplete or incorrect. We may deny your request, but we will provide a written explanation within 60 days.
You have the right to request confidential communications, such as contacting you in a specific way or sending mail to a different address.
You have the right to request restrictions on certain uses and disclosures of your information. While we are not required to agree to all requests, any agreed-upon restrictions will remain in effect until you request removal in writing.
You have the right to receive an accounting of disclosures of your health information.

We may use and disclose your health information for treatment purposes, including sharing information with other healthcare providers involved in your care.
We may use and disclose your information to operate our practice, improve care quality, and contact you when necessary.
We may use and disclose your information to bill for services and obtain payment from insurance providers or other entities.
Due to the open nature of some treatment areas, incidental disclosures may occur. If you prefer a private discussion, please notify our staff.
We may disclose information to help with public health and safety issues, research, or when required by federal or state law.
We may disclose information to medical examiners, funeral directors, workers’ compensation programs, law enforcement, or government agencies as permitted or required by law.
We may disclose information in response to legal proceedings, court orders, or subpoenas.
In emergencies, we may notify family members or authorized individuals.
We may contact you regarding appointments, scheduling changes, or office updates.
In the event of a change of ownership, your health information may be transferred to the new owner. You retain the right to request transfer of your records to another provider.
Substance use disorder records protected under 42 CFR Part 2 will not be disclosed without your written consent or as required by law.
